What are the requirements for application?

FOR REGULAR STUDENTS

  • Filipino Citizen
  • A Bachelor’s Degree or its equivalent obtained from a reputable school as evidenced by a Diploma and Transcript of Records. For graduates of Philippine private schools, the TOR must bear the Special Order from the Commission on Higher Education (CHED).  Photocopies/soft copies of the Diploma and TOR will suffice for application purposes.
  • Creditable performance in the National Medical Admission Test (NMAT) with a percentile score equal or higher than that prescribed by the Admission Committee. The NMAT must have been taken no more than two (2) years from the time of the application.
  • Certificate of GWA (General Weighted Average)
  • Certificate of Live Birth from the Philippine Statistics Authority (PSA)
  • Certificate of Good Moral Character from the Dean or Office of Students of the School previously attended (School Seal required)
  • 2 copies of 1.5 X 1.5 ID picture
  • Completely filled-up Application Form

FOR TRANSFER STUDENTS

  • Filipino Citizen
  • Transcript of Records from previous school up to last enrolled semester
  • Course description and clock hours of subjects taken in previous school
  • Honorable dismissal from the last school attended
  • Certificate of Live Birth from the Philippine Statistics Authority (PSA)
  • Certificate of Good Moral Character from the School previously attended (School Seal required)
  • 2 copies of 1.5 X 1.5 ID picture
  • Completely filled out Application Form

APPLICATION PROCEDURE- NEW/REGULAR FRESHMEN

FOR SY 2024- 2025

(Online Submission of Application Documents)

 

The CEU- SOM will be accepting applications for Year Level 1 of its Doctor of Medicine Program starting November 15, 2023.  The last day for submission of application documents is on July 30, 2024.  This application procedure applies to NEW/REGULAR applicants (applicants who have NOT previously been enrolled in another medical school); otherwise, please apply as a transfer student.

 

  1. Download the Application Form from https://www.ceu.edu.ph/admissions
  2. Completely fill up Application Form and submit to [email protected] with the subject: 
    APPLICATION-MED-YOURSURNAME,FIRSTNAME (example: APPLICATION-MEDNEW-DELACRUZ,JUAN)  Please check the box þRegular Student in the Application Form.
  3. Together with the Application Form, attach scanned copies of the following Credentials:
    Copy of Transcript of Records or True Copy of Grades
    Copy of Diploma or Certificate of Graduation 
    Copy of NMAT Examinee Report Form (NMAT Score) 
    Certificate of General Weighted Average
    Certificate of Good Moral Character from the Dean or Office of Students (School Seal required) 
    Certificate of Live Birth from the Philippine Statistics Authority (PSA)
    1.5 X 1.5 ID Picture
    Original copies of the foregoing will be required once accepted as a regular Year Level 1 student. 
  4. The CEU- SOM strictly adheres to the CHED Guideline that only applicants who have NMAT ranking of at least the 40th percentile may be accepted into a Philippine College of Medicine. 
  5. Submit an Essay to answer the questions in the attached interview schedule. 
  6. Wait for the Acknowledgement of Application Documents from the School of Medicine.  The email will indicate your Application/Student/Reference Number.  Please use this number to pay for the Application Processing Fee (PhP500). 
  • Payment may be accepted over-the-counter to any branch of the following: 
    - Security Bank (Use Payment Slip. Cash and check payments accepted.)
    - Philtrust Bank (Use Deposit Slip. Cash and check payments accepted.
    - MLhuillier (Use Payment Slip. Cash payment only.)
  • Write complete student name, student number and campus (Manila) in the deposit/payment slip and keep a validated copy for reference.
  • Please refer to the CEU website, www.ceu.edu.ph for other payment options.

     7. Submit a scanned copy of the Deposit/Payment Slip to [email protected] with the subject:
        APPLICATION-MEDNEW-YOURSURNAME,FIRSTNAME-APPLICATIONFEE

     8. A face-to-face interview will be scheduled once your Application Documents have been evaluated.
     9. Wait for the letter from the Admissions Committee regarding the outcome of your application.

Only complete applications will be processed.

APPLICATION PROCEDURE- TRANSFER STUDENTS

FOR SY 2024- 2025

The CEU- SOM will be accepting applications for transfer to Year Level 1 and Year Level 2 of its Doctor of Medicine Program starting November 15, 2023.  The last day for submission of application documents is on July 30, 2024.  This application procedure applies to TRANSFER applicants (applicants who have previously been enrolled in another medical school). 

 

  1. Download the Application Form from https://www.ceu.edu.ph/admissions
  2. Completely fill up Application Form and submit to [email protected] with the subject: 
    APPLICATION-MEDTRANSFER-YOURSURNAME,FIRSTNAME (example: APPLICATION-MEDTRANSFER-DELACRUZ,JUAN)Please check the box Transfer Student in the Application Form.
  3. Together with the Application Form, attach scanned copies of the following Credentials:
    Copy of Transcript of Records or True Copy of Grades from the previous medical school attended, up to the last enrolled semester
    Certificate of General Weighted Average from the previous medical school attended 
    Copy of NMAT Examinee Report Form (NMAT Score) 
    Certificate of Honorable Dismissal from the previous medical school attended
    Certificate of Live Birth from the Philippine Statistics Authority (PSA)
    1.5 X 1.5 ID Picture
    Course description and clock hours of subjects taken in previous school
    Original copies of the foregoing will be required once accepted as a transfer student.
