Admissions Procedure for Incoming First Year

  1. Go to the Guidance and Counseling Department located at SDV Bldg., 3rd Flr., Room 303 and get Application for Admission Form.
  2. Fill out the form (black/blue ink) and attach a 1.5x1.5 ID picture.
  3. Pay the corresponding examination fee at the Cash Department.
  4. Submit the accomplished application form with picture and receipt for examination to the Guidance and Counseling Department.
  5. Take entrance examination at the Guidance and Counseling Department.  Registration for examination is at least 15 minutes before the chosen schedule as specified below:

  1. Claim the examination result (pink form) from the Dean's Office.
  2. Prepare the necessary requirements for admission:
  • Original Form 138/High School Report Card/PEPT or ALS certification from DepEd
  • Good Character Testimony from the high school principal
  • NSO Birth Certificate (original and photocopy)
  • Entrance Exam Result (pink form)
  • Filled out Application for Admission Form
  • Recent 1.5 x 1.5 I.D. picture (2 pcs.)

       8. Return to the Dean's Office to present the requirements for admission/enrollment. If admitted, sign Student’s Pledge.

*For those who (a) stopped schooling after high school, (b) have taken the Philippine Educational Placement Test (PEPT) or (c) Alternative Learning System (ALS) from the Department of Education (DepEd), please go first to the Dean’s office for the initial interview and referral for examination.  Present the referral from the Dean to the Guidance and Counseling Department before taking the examination and follow step 1 to 8 procedures.

Admissions Procedure for Foreign Students

Resident Aliens

  1. Submit the necessary requirements to the Student Records Management Department (SRMD) window 5 for verification:  I - card
  2. Proceed to the Dean’s office for initial interview and referral for examination.
  3. Go to the Guidance and Counseling Department located at SDV Bldg., 3rd floor, Room 303 and get the Application for Admission Form.
  4. Fill out the form in black/blue ink and attach a 1.5 x 1.5 I.D. picture.
  5. Pay the corresponding examination fee at the Cash Department.
  6. Submit the accomplished application form with picture, examination fee receipt and referral for examination at the Guidance and Counseling Department.
  7. Take the Entrance Examination at the Guidance and Counseling Department.  Registration for examination is at least 15 minutes before the chosen schedule as specified below:

Filipinos who Studied Abroad

  1. Submit the necessary requirements to the Student Records Management Department (SRMD) window 5 for verification:
  • Transcript of Records authenticated by the Philippine Consulate abroad, except those issued by Philippine Schools abroad
  • NSO Birth Certificate (original and photocopy) 
  1. Proceed to the Dean’s office for initial interview and referral for examination.
  2. Go to the Guidance and Counseling Department located at SDV Bldg., 3rd floor, Room 303 and get the Application for Admission Form.
  3. Fill out the form in black/blue ink and attach a 1.5 x 1.5 I.D. picture.
  4. Pay the corresponding examination fee at the Cash Department.
  5. Submit the accomplished application form with picture, examination fee receipt and referral for examination at the Guidance and Counseling   Department.
  6. Take the Entrance Examination at the Guidance and Counseling Department.  Registration for examination is at least 15 minutes before the chosen schedule as specified below:

  2.    Certificate of Graduation
  3.    Police Clearance
  4.    Photocopy of Passport
  5.    Passport size I.D. picture (5 pcs.)

           *Letters a, b and c requirements should be authenticated or certified by the Philippine Consulate (Country of Origin/Abroad)

  1. Proceed to the Dean’s office for initial interview and referral for examination.
  2. Go to the Guidance and Counseling Department located at SDV Bldg., 3rd floor, Room 303 and get the Application for Admission Form.
  3. Fill out the form in black/blue ink and attach 1 pc. 1.5 x 1.5 I.D. picture.
  4. Pay the corresponding examination fee at the Cash Department.
  5. Submit the accomplished application form with picture, examination fee receipt and referral for examination at the Guidance and Counseling Department
  6.  Take the Entrance Examination and Psychological Test (or the Qualifying Examination for Dentistry Proper, if applicable) at the Guidance and Counseling Department. Registration for examination is at least 15 minutes before the chosen schedule as specified below:

  1. After taking the examination at the Guidance and Counseling Department, proceed to the International Languages Department to take the English Proficiency Test.
  2. Claim the examination result (pink form) (or the Dentistry Qualifying Examination result, if applicable) from the Dean’s office; and psychological test result and English Proficiency Test result from the International Languages Department.
  3. Prepare the following additional requirements for admission:
  •    Entrance Exam Result (pink form), psychological test result, English Proficiency Test result (or the Dentistry Qualifying Examination Result, if applicable)  
  •    Filled out Application for Admission Form
  •    Recent 1.5 x 1.5 I.D. picture (2 pcs.)
  1. Return to the Dean’s office for recommendation of acceptance and proceed to the Student Records Management Department (SRMD) for approval of acceptance and enrollment.

 

Admissions Procedure for Second Degree

  1.  Submit a copy of the Transcript of Records and the Transfer Credential to the Dean’s office for initial interview and referrals for Examination and Drug Assay Test.
  2.  Proceed to the Guidance and Counseling Department located at SDV Bldg., 3rd floor, Room 303 and get the Application for Admission Form.
  3.  Fill out the form in black/blue ink and attach a 1.5 x 1.5 I.D. picture.
  4.  Pay the corresponding fees for Examination and Drug Assay Test at the Cash Department.  
  5.  Submit the accomplished application form with picture, examination fee receipt and referral for examination to the Guidance and Counseling Department.
  6.  Take the psychological test (or the Dentistry Qualifying Examination, if applicable) at the Guidance and Counseling Department.  Registration for examination is at least 15 minutes before the chosen schedule as specified below:

  1. After taking the test, proceed to the Clinical Laboratory to undergo the Drug Assay Test.
  2. Claim the Result of the Psychological Test (or the result of Dentistry Qualifying Examination, if applicable) from the Guidance and Counseling Department and the result of Drug Assay Test from the Clinical Laboratory.
  3. Prepare the following additional requirements for admission:
  • Psychological Test Result and Drug Assay Test Result
  • Dentistry Qualifying Examination Result (for Dentistry Proper applicants only)
  • Certificate of Good Moral Character/Recommendation Letter from previous College/University
  • Filled out Application for Admission Form
  • Recent 1.5 x 1.5 I.D. picture (2 pcs.)
  • NSO Birth Certificate (original and photocopy, not required if graduated from CEU)

       10. Return to the Dean’s office for evaluation of subjects/units and approval of enrollment.

 

Admissions Procedure for Transfer Students

  1. Submit the necessary requirements to the Student Records Management Department (SRMD) window 1 for verification:
  • Transfer credential
  • Transcript of Records/Scholastic Records
  • Certificate of Good Moral Character
  1. Proceed to the Dean’s office for initial interview and referrals for examination and drug assay test.
  2. Go to the Guidance and Counseling Department located at SDV Bldg., 3rd floor, Room 303 and get the Application for Admission Form.
  3. Fill out the form in black/blue ink and attach a 1.5 x 1.5 I.D. picture (1 pc.)
  4. Pay the corresponding fees for Examination and Drug Assay Test at the Cash Department.
  5. Submit the accomplished application form with picture, examination fee receipt and referral for examination to the Guidance and Counseling Department
  6. Take the psychological test (or the Dentistry Qualifying Examination, if applicable) at the Guidance and Counseling Department.  Registration for examination is at least 15 minutes before the chosen schedule as specified below:

       12. Proceed to the offices of the Academic Department Heads for evaluation of General Education subjects/units.
       13. Return to the Dean’s office for final interview and approval of enrollment.

  1. An applicant for a Master's Degree should be a Bachelor's Degree holder with a weighted average of at least 2.00 or its equivalent. He/She should also be proficient in English and shall manifest evidence of creativity and research potential.
     
  2. An applicant for a Doctoral Degree should have a Master's Degree with a weighted average of at least 1.75 or its equivalent, have at least 2 years of teaching or administrative experience or its equivalent, be proficient in English and manifest evidence of ability to think clearly, logically and creatively.
     
  3. An applicant shall be required to submit an original transcript of records with Special Order (S.O.).Applicants from state colleges and universities shall be required to submit only the original transcript of records Transferees should present transfer credentials in addition to Transcript upon enrolment.
     
  4. New students and transferees shall be required to take the Graduate School qualifying exam. Only students who completed their Master's  Degree in CEU shall be exempted from the qualifying exam when they enroll in the doctoral program.
     
  5. The Graduate School Associate Dean shall interview the applicant and shall refer him/her to the School Dean concerned who, in turn, shall evaluate his/her scholastic records.
     
  6. Transfer applicants may be given credits for a maximum of nine (9) units in the Masters program or twelve (12) units in the Doctoral program of subjects taken from a reputable school upon advice and recommendation of the School Dean concerned.
