Admission Schedule in Manila

For the month of August & September - examinations are conducted on the 2nd and 4th Saturdays of the month with the following time schedules:

Application Procedure for Incoming First Year in Manila
  1. Proceed to the Counseling and Testing Department for the application for entrance examination.
  2. Submit accomplished application form and five (5) 1½” x 1½” ID photos to the Counseling and Testing Department (CTD), 3rd floor, SDV; get schedule of examination and room assignment.
  3. Proceed to the Cash Department and pay the entrance examination fee.
  4. Take entrance examination at the specified date and place of examination.
  5. While waiting for examination: results, prepare additional requirements for admission:
  • Original Form 138 / H.S. Report Card
  • Good character testimony from the high school principal
  • NSO copy of birth certificate (original and photocopy)
  • Claim test result from CTD and the accomplished application form for admission.
  • Certificate of Eligibility (for Dentistry applicants only)
  • Proceed to the Dean to present the requirements and for interview. If admitted, sign Student’s Pledge
  • ADMISSION REQUIREMENTS FOR:

    B.S. Nursing Applicant:

    • Height Requirement: Male - 5'2", Female 5 ft.
    • 82% and above 4th Year General Weighted Average
    • No Final Grade Lower than 80% in any Subject


    B.S. Accountancy Applicant:

    • 85% 4th Year General Weighted Average or its equivalent
     
    Application Procedure for Foreign Students in Manila

    Resident Aliens

    Pre-Enrollment Guidelines

    1. Submit requirements for verification at SRMD Foreign Students Section.
    2. Proceed to the Dean of the school/college you are applying to, for interview and recommendation.
    3. Follow appropriate procedure for enrollment as indicated above.

    Additional Requirement

    • I-Card



    Filipinos Who Studied Abroad

    Pre-Enrollment Procedure

    1.    Submit requirements to SRMD Foreign Students Section for verification of records.
    2.    Proceed to the Dean of the school/college you are applying to, for interview and referral for the entrance examination.
    3.    Follow appropriate procedure for enrollment as indicated above.

    Requirement

    • Photocopy of passport



    Non Resident Aliens

    Pre-Enrollment Procedure

    1. Submit requirements to SRMD Foreign Students Section for verification of records.
    2. Proceed to the Dean of school/college you are applying to, present English Proficiency Certification and undergo interview. If the student is unable to present English Proficiency Certification, he/she shall take Intensive English Lessons Part A.
    3. Get referral from the Dean to the Counseling and Testing Department for the entrance examination and to the English Department for the English Proficiency Test.
    4. Proceed to the Cash Department and pay the entrance and psychological examination fees. Those applying to the Doctor of Dentistry program, shall also pay the fee for the Qualifying Examination for Dentistry
    5. Take entrance and psychological examinations (and Qualifying Examination for Dentistry, if applicable) on the specified dates.
    6. Proceed to the English Department for English proficiency test and Interview.
    7. Claim test results and Application for Admission form from CTD. Claim also the English proficiency test result.
    8. Return to the Dean for recommendation of acceptance and proceed to the University Registrar for approval acceptance and enrollment.


    Admission Requirements

    1. Official transcript of records
    2. Certificate of good moral character
    3. Certificate of graduation
    4. Affidavit of support of the person supporting the applicant’s study
    5. Bank Certificate from the bank of the person funding the study of the applicant
    6. Police clearance
    7. Photocopy of passport
    8. Letter of application
    9. Personal history statement
    10. Appropriate visa


    Note: The first six requirements should be authenticated by the Philippine Consulate abroad.

     
    Application Procedure for Transferees in Manila
    1. Proceed to the Dean of the school/college for submission of requirements and interview.
    2. Go to the Cash Department for payment of fee for drug test.
    3. Take drug test at the Clinical Laboratory within the day of payment.
    4. Claim drug test result at the Dean's Office on the designated date.
    5. Accomplish Application for Admission form
    6. Pay Entrance Examination fee at the Cash Department. Those applying for the Doctor of Dentistry program shall also pay the fee for the Qualifying Examination for Dentistry.
    7. Proceed to CTD to get schedule of examination(s) and submit application form.
    8. Take the examination at the CTD Testing Center, 3rd floor, SDV.
    9. Get results of examination(s) and accomplished application form from CTD.
    10. Return to the Dean’s Office for evaluation of professional subjects.
    11. Proceed to the offices of the Academic Department Heads for evaluation of General Education subjects/units.
    12. Return to the Dean’s Office for final interview and approval of enrollment. If admitted, sign Student’s Pledge.
    13. Follow enrollment procedure as in New Students. Documents to be submitted to SRMD are: Application for Admission, letter seeking admission with parent’s/guardian’s signature, transfer credentials, scholastic records, transferee’s and parent’s/guardian’s waiver, drug test results, pictures, birth certificate, character testimonies, signed Student’s Pledge, curriculum checklist, Adviser’s Form, Routing Form, and I-Card (for aliens).


    Requirements for Admission for Transferees

    • Transfer credentials
    • Original scholastic record bearing school seal and signature of registrar
    • Good character testimony from an authorized official of former school
    • Student’s letter seeking admission to CEU with parent’s/guardian’s signature
    • NSO copy of the birth certificate (original and photocopy)
    • Five (5) copies of latest ID photo (1½” x 1½”)
    • Transferee and Parent’s/Guardian’s Waiver
    • For applicants to the School of Dentistry :Certificate of eligibility
