WHAT TO DO IN CASE YOU

  • Inform the guard and surrender any identification document before entering the gate;
  • Secure a temporary I.D. form from the Student Affairs Office;
  • Write to your Dean asking approval to apply for a new I.D.;
  • Proceed to the Accounting Office for encoding;
  • Pay at the Cashier;
  • Proceed to the I.C.T. for your picture taking;
  • Claim your new I.D. from your School/College.
  • Inform the guard and surrender any identification document before entering the gate;
  • Secure a temporary I.D. form from the Student Affairs Office and go to the said guard to present the temporary I.D. and claim identification document surrendered.

IMPORTANT:  After having obtained a temporary I.D. form five (5) times for lost I.D./I.D. left at home (if no violation has been committed), a student entering school without an I.D. shall be dealt with according to the University rules and regulations on wearing the school uniform/I.D.

  • Prepare a letter addressed to the Dean;
  • Proceed to the Accounting Department for encoding;
  • Pay at the Cashier;
  • Present your receipt to the: - Student Records Management Department for Certified True Copy

                                                                - Accounting Office for the Duplicate Official Receipt

  • Present your C.O.M. at the Accounting Department for encoding;
  • Pay at the Cashier;
  • Present your receipt to the Accounting Department and get your duplicate permit.
  • Secure the form from the Dean’s Office and have it completely filled-out;
  • Submit the form and request for the Dean’s approval;
  • Proceed to the Accounting Office for encoding;
  • Pay at the Cashier;
  • Present the receipt to your School/College for the examination schedule.
  • Proceed to the Student Affairs Office to secure a temporary I.D. form to avoid further violation;
  • Claim your confiscated I.D. from your School/College within five (5) days from the date of confiscation.
  • Secure the form at the Accounting Department and have it completely filled-out;
  • Follow the procedure indicated in the form;
  • Pay at the Cashier;
  • Go back to the Accounting Department for your permit

IMPORTANT:  Promissory Note is good only for Prelim and Midterm periods.
